COVID-19 Vaccination Registration

MCPH is happy to offer these special vaccination events based on current availability of COVID-19 vaccine. Before we can process your event registration we need to either locate your existing patient account or create a new account for you.

If you have already received a COVID-19 vaccination from any MCPH event, and are returning for your 2nd injection, you may already have an existing account. If you know the username and password associated with your account that you use to access the patient portal you may use that information to sign in below.

If you have NOT received your first COVID-19 vaccination from Miami County Public Health, and this is your first time signing up, please skip to the "Create Account" button at the bottom of this page.
